What Is the Difference Between a Generic Drug and a Biosimilar?

Generic medicines and biosimilars both expand access after exclusivity ends, but they are not approved in exactly the same way. This explainer clarifies the difference, why complex biological medicines cannot be copied identically, and what equivalence means for patients.

When exclusivity ends, other manufacturers may offer lower-cost versions of a medicine. For conventional small-molecule drugs, these are usually called generics. For complex biological medicines, the corresponding products are called biosimilars.

What is a generic drug?

A generic drug uses the same active ingredient, strength, dosage form, and route of administration as an approved reference medicine. Regulators require evidence that it delivers the medicine into the body in a sufficiently similar way.

What is a biosimilar?

A biosimilar is a highly similar version of a biological medicine made in living systems. Because biological products are large and complex, no manufacturer can reproduce every molecular detail exactly.

Why are the approval pathways different?

Generic approval focuses heavily on pharmaceutical equivalence and bioequivalence. Biosimilar approval requires extensive analytical comparison, and may also include functional, pharmacokinetic, immunogenicity, and clinical evidence.

Does similar mean less effective?

No. An approved generic or biosimilar must meet regulatory standards showing that clinically meaningful differences are not expected. That does not mean every patient experience will be identical, because formulations, delivery devices, and individual responses can vary.

What should patients check?

Patients should confirm the exact product, dose, delivery device, and approved indication with a clinician or pharmacist. Substitution rules may differ by country and by medicine.
